I consent to the electronic delivery of documents. The documents subject to this consent include, but are not limited to, the ADV Part II A & B, Privacy Policy, and financial statements.

I understand that I will gain access to the SGI web access portal to retrieve the above aforementioned documents, and I confirm that I have such web portal access.

I acknowledge most documents posted to the website will be in Portable Document Format (PDF).

I acknowledge that I may at any time request and receive at no charge, a paper copy of any specifically requested documents by contacting: Summit Global Investments, LLC 620 S. Main Street Bountiful, Utah 84010

I understand that any personal information I provide to SGI, including email addresses and other information will be used solely for the purposes of conducting business with SGI.

I understand that my consent may be revoked or altered, including any change in the electronic mail address I have provided, at any time by notifying SGI of such by telephone, regular mail, or electronic mail.

I understand and agree that this consent will remain in effect for the entire time I am a client or until I revise and or revoke my consent to electronic delivery.
